Snapshot of a single MSFT_PhysicalDisk row used by ProxmoxDiskFilterResolver. Captured into a POCO so the resolver can be unit-tested without WMI.

UniqueIdFormat values per MSFT spec: 0 = Vendor Specific, 1 = Vendor Id, 2 = EUI64, 3 = FCPH Name, 8 = SCSI Name String. Formats 1, 2, 3, 8 are the ones that yield stable WWN-like identifiers matching udev ID_WWN.
